The Squallshaper's Hat was only available as potential loot during the special pre-Mists of Pandaria expansion event, Theramore's Downfall (scenario). This was the level 85 version and should not be confused with the level 90 version currently available in MoP.

The headpiece uses the wizard hat model. It is soft blue with a somewhat-decorated golden band around its base. For those looking for this specific model and color, try  Mistscape Wizard Hat . Best keep an eye on the Auction House, however - it has a low drop rate.
